Over the years, I’ve seen how obesity doesn’t just affect weight — it silently damages other organs, especially the kidneys. For patients with advanced chronic kidney disease, losing weight is often one of the biggest challenges when preparing for a transplant. Now, research from Mayo Clinic has brought encouraging news: undergoing a sleeve gastrectomy before a kidney transplant can significantly improve kidney function and even increase the chances of receiving a transplant before dialysis is needed.
In this study, patients with a body mass index (BMI) over 35 who underwent sleeve gastrectomy not only achieved meaningful weight loss, but also showed improvements in their estimated glomerular filtration rate (eGFR) — a key measure of kidney health. On average, their kidney function improved within two to six months after surgery, while those who didn’t have the procedure saw no such change.
Even more importantly, nearly one in four patients who had the surgery were able to receive a kidney transplant without starting dialysis, compared to none in the group that did not undergo surgery. This matters because delaying dialysis, or avoiding it altogether, is linked to better transplant outcomes, faster recovery, and longer graft survival.
In addition to better kidney health, patients who had sleeve gastrectomy required fewer medications for diabetes and high blood pressure, two major risk factors that often complicate transplant readiness. For some, this surgery even led to improved diabetes control or complete remission.
From my perspective as a bariatric surgeon, this study highlights a crucial point: treating obesity before it causes further damage can open doors to life-saving treatments sooner. Sleeve gastrectomy is not just about losing weight — it’s about giving patients with kidney disease a better chance at transplant success and a longer, healthier life.
